Enrollment at Prussing Elementary School Please carefully read the sections below that will describe all of the documents that are required to enroll your student at a CPS school. Please note that if you live within the Prussing attendance boundaries, your child will be automatically accepted for enrollment at Prussing School.
Prussing is an "open enrollment" school. An open enrollment school is a neighborhood school without a magnet program that accepts students who live within its attendance boundaries or who have applied and been accepted to our school via the Office of Access and Enrollment's "Options for Knowledge Lottery Program." Please note that "open enrollment" does not mean that we are capable of taking any student who wishes to attend Prussing. If you have questions about the lottery process please contact [email protected] or call the Office of Access and Enrollment at 773-553-2060. At this time, we do not currently have any open enrollment seats available.

Enrollment Requirements:
Per Board Policy 08-0827-P02; The parent, legal guardian, or temporary custodian of any elementary school-aged child seeking enrollment in a Chicago Public School for the first time shall present:
1. Proof of age:
All students must have a physical examination within one year of entry, which includes an UPDATED immunization record per the requirements stated below, due upon enrollment, no later than October 15th of the current school year:
All students in Kindergarten, 2nd, and 6th grade must have a dental exam completed by a licensed dentist, due upon enrollment, no later than May 15th of the current school year.
5. Vision Examination Requirements:
Entering an Illinois school for the first time at any grade level or who are entering Kindergarten. To view and print the IL eye exam form, go to the Parents tab on our website's home page and click on Forms. Please sign and date the form.

Student Transfer Following Change in Residence
Elementary and Middle School Students
Absent extenuating circumstances, transfers of elementary and middle school students, whose parents/guardians change their place of residence to a new attendance area, shall be made at the end of the school year, provided the distance factor does not adversely influence the students' safety, attendance, and academic progress. Parents who immediately want to transfer their child(ren) to an attendance area school after moving from one attendance area to another shall be allowed to do so.
TRANSFERS (not your child's first enrollment)
From another CPS school
In order for a student to be transferred from one CPS school to another the student must be released from the school previously attended. The parent/guardian of the student must contact the previous school and request that their student be released before coming to Prussing School for enrollment. You must bring all of the documentation, as stated above, along with a transfer form and cumulative record from your child's previous CPS school. Your child's previous school should have copies of all of your child's medical records and should provide you with all of the above documentation, upon request.
If your child has an IEP (Individualized Education Plan) or a 504 plan you will also need to bring that document with you upon enrollment. Lastly, you MUST provide us with TWO original, current proofs of address proving that you currently reside within our attendance boundaries or proof that your child was accepted into our school via the Options for Knowledge Program from the Office of Access and Enrollment along with TWO current proofs of your current Chicago address.
From a Private School (including Home School), Foreign School,Charter School or Other School District
You must bring all documentation as stated above for proof of age, proof of address, health requirements, and all supplementary documents that you received from the prior school upon transfer, including but not limited to an IEP or 504 plan, if applicable. You are also required to provide "proof of good standing" for your child. See details below:
Proof of Good Standing
Please note the following: per CPS Board Report 10-0623-P01, Section 702.1,
"Students transferring from another Illinois public school must produce the Illinois State Board of Education "Student Transfer Form" completed by their former school verifying that they are "in good standing" and are not currently being disciplined by a suspension or expulsion. Failure to provide this form will result in the denial of enrollment of the transferring student." Please see hyperlink at the bottom of this enrollment page for the required form.
Grade Placement
"Students who were previously enrolled in a private school (including home school), foreign school, Charter school or other school district who seek enrollment in a Chicago Public School are subject to transcript evaluation to determine proper grade placement and also may be evaluated to verify appropriate grade placement."

Please note: As of January 6, 2023, the Illinois State Board of Education (ISBE) is no longer accepting homeschool registration forms through email or by physical mail. Our CPS Homeschooling website has been updated to reflect this new online Illinois State Board of Education Notification of Intent to Homeschool form. The requested CPS Documents remain the same (Statement of Assurance and Letter).

- Dropping of the completed enrollment packets (medical paperwork due by Oct 2025)
- Verifying residency (2 pieces)
- Making a copy of the Original Birth Certificate (must be presented at time of enrollment drop-off)
- Paying school fees: $50.00 annually (Maximum $100 per family). Payable to Prussing Elementary; ePay is preferred for tax and receipting purposes. If paying cash or check, please speak to the Main Office directly

- Current utility bill (electric, gas, water bill connected to your home address)
- Valid Illinois driver’s license or State of Illinois identification card
- Notarized lease that states beginning and ending dates of lease period, if you are renting.
- Deed
- MediPlan/Medicaid card
- Court documents
- Illinois Department of Public Aid card
- Stamped United States Post Office change of address form
- Illinois state aid check/social security check
